STEP 1: Please fill out the form below completely, and click "SUBMIT VENDOR APPLICATION".
STEP 2: Payment. 
  • TO PAY BY CASH OR CHECK: Make checks payable to "Mosaic Church". Cash or checks can be mailed or dropped off at Mosaic Church, 701 Cole Dr. SW, Lilburn, Ga, 30047. If dropping off, please contact us by phone or email (listed at the top of this application) to be sure someone will be there to meet you.
  • TO PAY ONLINE: At the bottom of this page is a tab labeled PAY VENDOR FEES. When you click on it, you will be redirected to the Mosaic Giving Portal. There will be a drop-down menu by the word "Fund." Please select "Holiday Bazaar - Vendor Fee" in the drop-down menu.
